WYCF Final Report Instructions

Grantees should follow these instructions for completing the final report to close out Wyoming Community Foundation programmatic and general operating support grants.

When should your report be submitted? Your final report is due within one year from the date your organization receives notification of the grant award. Officially, this is the day your award letter is dated. Please note that in order to be eligible to reapply for funds through the WYCF, previous grants must be closed out which may mean submitting a final report in less than 12 months.

Which report format should you use? Your original application should indicate if your request was for a programmatic grant or operating support grant. If you are unsure what type of grant your organization received, please contact the WYCF program staff and they will be happy to assist you. Formats for both types of grants follow.


PROGRAMMATIC GRANTS
Please use the following format when completing your final report. All final reports should be typed, clearly labeled and include a financial report.

Section 1: Grantee Information
Organization: the name of your organization or if not a 501 (c)(3), the name of the fiscal sponsor your group used to submit your proposal.
Project Title:  Name of the project as indicated on your application.
Grant Number: Found on the first page of your award letter this is an 8 digit number assigned to you by the WYCF.
Grant Amount: Amount of the grant issued to your agency.
Grant Period: Beginning of the grant period is the date on your organization’s award letter. End of the grant period can be calculated by simply adding 12 months to the award letter date.
Report Due: This date corresponds to the end of the grant period. If you received an extension please indicate the extended due date.
Report completed by: Name of individual completing report.
Telephone or email: Contact information for individual completing report.
The information contained in this report, including all attachments, is accurate and complete. Include this statement with the appropriate signature.

Section 2: Narrative
Please provide detailed answers to the following questions.
  1. Did this grant enable your organization to leverage any additional funds?
  2. What were two major challenges the project or organization faced in implementing its work during the grant period and how were these challenges addressed?
  3. Did the organization require any technical assistance or midcourse corrections with the proposed program? If so, how was the change affected?
  4. Will this project continue beyond the grant period? If so, how?
  5. What measurable impact did this project have on the community? Respond relating to the goals and outcomes outlined in your original application.
  6. Address any items listed as a special condition of your grant.

Section 3: Financial Report
Include the attachments listed below.
  1. Income Statement for the period ending with the program’s completion/grant funds being expended
  2. Summary statement listing all expenditures paid with grant proceeds
  3. Copies of all invoices paid with grant proceeds
  4. Please include a reimbursement check with the final report for the balance of any unexpended funds from the grant. Checks should be made out to the Wyoming Community Foundation.

Please use the following format when completing your final report. All final reports should be typed, clearly labeled and include a financial report.

  3. How does your agency plan to cover general operating costs in the future?
  4. Share any additional information regarding the use of this grant money the WYCF would find interesting or helpful.
  5. Address any items listed as a special condition of your grant.

Section 3: Financial Report
Include the attachments listed below.
  1. Original budget as submitted with the grant.
  2. Any budget amendments approved through the year.
  3. Budget to actual income statement for the period ending with the grant funds being totally expended.
  4. Please include an explanation if grant funds were applied to any specific line items.
